It was a holiday tragedy near New York's Times Square when a 60-year-old pedestrian lost her life after a chunk of building facade fell off and hit her. She was identified as Erica Tishman, a prominent architect who worked nearby. The accident happened in the heart of a neighborhood packed with thousands of shoppers and tourists. In April, city inspectors say they found "damaged terra cotta" on the facade of the 17-story building.
